This is a guide designed to help individuals with spinal cord injuries start families and to experience sexual pleasure. Given that sex and intimacy are integral aspects of adult life, there is no reason why someone with a spinal cord injury should be excluded from these essential experiences.
Welcome.
I was injured in 2014 and paralyzed from the belly button down. For those of you who understand the spinal cord injury designations, I am a T8-T9 complete paraplegic, ASIA Grade A.
Let’s get straight to the point. You’re a guy, you’ve been injured, and you miss what you had before: sexually, or at least the ability to finish. Well, you’re in luck, you “came” to the right place . There is a complete lack of information on this subject on the internet. I personally had to research this stuff for 3 years to finally make this happen. Once I figured it out, I was able to finish at a 90-100% rate on a daily basis if I wanted to (but it’s a lot of work and my method can be taxing on my back so I don’t do it every day. Max once per week). Good news, It is entirely possible.
There’s a few reasons a paralyzed person might want to make this happen. Maybe, like me, you’re trying to get your wife pregnant and start a family. Or maybe you’re a normal guy and looking for a hormonal release. Totally normal. I would say both applied to me.
First off, I want to make it clear. I’m not a doctor, this is not medical advice, and if you have any doubts or confusion, always ask a doctor in every circumstance. None of what I’m about to tell you has been thoroughly researched or studied and is entirely based on anectdotal research and testing I had to do myself which took me 3 years to figure out. All I’m doing is telling you what worked FOR ME. So if you burn yourself, or get a pressure sore, or get injured or something like that, it is on you.
SO LET’S GET STARTED. You’re going to need a few products to make this happen. I do not sell these products or manufacture these products myself. Below you will find a list of the products you’ll need- some mandatory, some optional.
Once you are familiar with the products, you can scroll down and I’ll give you a step by step guide on how to make this happen.
(2-Two)Tens 7000 Units – MANDATORY
The TENS 7000 which I bought on Amazon is a device designed to induce the ejaculation reflex controlled by the autonomic nervous system . This product utilizes advanced Transcutaneous Electrical Nerve Stimulation (TENS) technology to provide controlled electrical stimulation.
I highly recommend specifically buying the Tens 7000 Unit, and not one of the other units out there.
You specifically need a “Tens” unit which delivers a constant flow of electricity in “Normal” mode. Most of the other units out there are overly expensive and gimmicky and didn’t work for me.
Why do you need 2 of these units?
For me, 1 was rarely enough while two almost always got the job done.
Some people are able to finish with 1 unit.

(1-One) Set of Conductive Rubber Loops – MANDATORY
Conductive Rubber Loops are made for use with the TENS 7000 unit, an electrostimulation device. Designed for comfort, these loops offer an effective solution for electroejaculation therapy. They attach securely to the TENS 7000 wires, providing targeted stimulation without the need for the annoying sticky squares which come with the unit. The soft and flexible construction contours to the body, ensuring a personalized and gentle experience.
The link below is where I got my conductive rubber loops. I’ve tried out different types of loops and these are the best with the highest conductivity.
(1-One) Set of Rechargeable Batteries- MANDATORY
You need rechargeable batteries because you want to have a full charge on the batteries every time you use them. If you buy non-rechargeable batteries, it is going to get very expensive for you. Cheaper is always better in my opinion.
The link below is where I got my rechargeable batteries. These seem to work great. The kit comes with a charger and 4 9-volt batteries.
(1-One) Reading Pillow – OPTIONAL
I use the reading pillow for the “W” Position which I explain below.
Basically, I put the reading pillow under my butt with the legs sticking out to give myself a little extra stability and support and to not over-strain my legs.
(1-One) Ferticare 2.0 Vibrator- OPTIONAL
As you’ll see in the guide below, I use the Ferticare 2.0 as sort of a back up plan in case the Tens Units don’t get the job done.
I would not recommend starting out with the Ferticare. It is extremely expensive and not really necessary.
If you have the money and want to buy it, go for it, but the tens units by themselves usually will get the job done. If you want to add a vibrator, consider starting with the Hitachi Magic Wand, because it is much cheaper.
With the added addition of the Ferticare I am able to finish about 90% of the time. Without the Ferticare, that number goes down to about a 70-90% success rate.
(1-One) Artificial Insemination Kit – OPTIONAL
If you’re trying to get your spouse pregnant, there are a lot of kits out there. This one is technically for dogs but it worked fine for us. It’s also pretty dang cheap which is always a good thing. You can pay up to $300 for some of these kits but in my opinion they all do the same thing so might as well get a cheap one.

Warnings and Risks
As I mention many times in this guide. I’m just explaining to you what worked for ME. Obviously there haven’t really been any medical studies on any of this and there are ways you could hurt yourself or cause damage. Here’s a list of the issues I can think of:
- The W Position: I’m not 100% sure the W position caused me to break my femurs. But I’d say I’m 80% positive. Unfortunately the W position increases your chance of ejaculation success substantially. Still, I would be very careful with the W and not do this every day, or even every week. Maybe once a month max. It might even be worth getting an X ray or bone scan to make sure your bones are strong. Either way be careful, this position puts a lot of downward pressure on your hips and femurs and if you’re unlucky like me, this may result in bad things.
- Autonomic Dysreflexia: a dangerous syndrome involving an overreaction of your autonomic nervous system. It causes a sudden and severe rise in blood pressure, in addition to other symptoms. If you are at risk for autonomic dysreflexia, make sure to speak with your doctor or a professional before trying this. I do not get AD but I have heard that successful ejaculation or even electricity can induce AD, so you ned to be EXTREMELY CAREFUL and definitely CONSULT A PROFESSIONAL BEFORE TRYING.
- Electrical Burns: Sometimes I get minor electrical burns from the process in the form of tiny red dots. These tend to go away in 1-2 days. Obviously you want to take care of your skin always so make sure to do more research if you’re worried about it.
- Edema and Swelling: If you go too crazy with the ferticare and apply to much pressure or use it for too long, you can cause swelling or edema of the penis. This is why I recommend using the ferticare towards the end of the protocol on your last 2 or 3, 5 minute tries, max.
- Back Pain: I have experienced back pain from sitting in the “W” position for long periods of time. Generally my way of dealing with this was to go to the local park and hang from a pullup bar. I personally have spinal fusion from T8-T9, so any time you put pressure on the rods and pins in your spinal cord, you are taking a risk. Again, make sure to ask the appropriate professional before you attempt any of this. I’m just telling you what worked for me.
Setting Up the Loops
You’re going to want each loop to have 2 of the same color connected to it. So with 4 loops you’re going to have the following setups:
- Black/Black
- Black/Black
- Red/Red
- Red/Red
It’s going to look something like this:
Setting Up the Tens Unit
- The knobs on your tens unit are going to be turned to max. On my Tens 7000 unit this is at level 8 on both knobs.
- The “Mode” is going to be set to “Normal-” do NOT use burst or any of the other modes, they didn’t work for me. Just stick with “Normal.”
- The “Width” or the pulse width is going to be set to 300. To change the width, push the “set” button once.
- The “Rate” is going to START at 50hz. Key words- START AT. To change the rate, push the “set” button twice.
Body Positioning – The “W” Position- WARNING!
WARNING!!! Please read!!!!!
I was tempted to take this entire guide down because of the W position. Since I made this guide, I fractured both of my femurs I believe as a result of overusing the W position. Please consider that as an individual with a spinal cord injury, your bones are weaker than a normal person’s. So while the W position does increase the chances of success substantially, you may be putting yourself at extreme risk. I will need major surgeries most likely to repair the damage done. Consider only using the W position very occasionally, perhaps to get a sperm sample to take to a lab. Consider trying all of this laying on your back instead. I would say the fractures were not worth it for the success that comes with the W position. Please, please, please, be careful.
What I am about to say may be very difficult for you to achieve depending upon your injury level. I personally am a “para” or a paraplegic. For someone who is a “quad” or a quadraplegic, I am not sure how you would go about getting into the W position. You may actually not need to. From what I’ve been told, achieving ejaculation/orgasm can be much easier for a quad, and you may not need to get into the W position. However, for me, the W position is extremely important– I cannot finish without it.
I want to make it EXTREMELY CLEAR. The W position may be dangerous. Sometimes after sitting in the W position for an extended period of time I get back pain.
***NOTE: For me personally, I sometimes have to go to a local park with a pullup bar and hang from the bar and do pullups to make this pain go away. The downward pressure on the spine from sitting in the W position can be a lot, though for me, it has not ultimately been a problem***
Again, THIS IS NOT MEDICAL ADVICE, ALWAYS TALK TO A DOCTOR. If you have titanium hardware in your spinal cord and spinal fusion like I do, you may want to be careful with the W position.
All this being said, as mentioned, I personally cannot finish without being in the W position. I believe it has something to do with stretching the quads and inducing muscle spasms. Muscle spasms seem to be important in achieving the ejaculation reflex. For me, laying down on my back doesn’t work very well- it has a very, very low success rate, though I have achieved ejaculation from the laying down position (rarely).
As mentioned in the “products list” above, I recommend getting a reading pillow to put under your butt, though it’s optional.
If you have to use the laying down position, I have read that putting a pillow or two under your butt or laying on a harder surface to cause spasms to occur helps.
Loop Positioning on the Shaft
From the bottom of the shaft to just below the tip, you’re going to want to position the loops in the following order: black, black, red, red.
Make sure to put the top loop BELOW THE TIP, NOT ON THE TIP- AS YOU PROBABLY REMEMBER FROM BEFORE YOU WERE INJURED, THE TIP IS VERY SENSITIVE.
The setup will look like this:
Erections: Using Viagra, Cialis, Trimix, Quadmix, Caverject, Etc.
In my experience, it is better to take something to induce an erection if you are able to do so. My success rates tend to be much higher if I have taken a pill (in my case just 1 cialis will do the trick, nothing crazy).
Even if the erection goes down at times while attempting this, the added addition of cialis or viagra seems to help get me to the finish line. Without cialis or viagra it can be hit or miss sometimes but with, it seems to work more.
It is important to note though, it is entirely possible to ejaculate without an erection. I have even heard of some people having more success without an erection. Everyone is different.
Bowel and Bladder
I recommend clearing out anything in bowel and bladder. Not because you’ll have an accident, but because I noticed my success rate is higher when everything is cleared out.
The Protocols – The Timing – The Power
Alright so this is the important part of the guide.
Note: I would recommend if you get through more than 4-5 sets with no results, and you are using the “W” position which I think is really important, try rocking back and forth and bending forward- lean forward and lean backwards some to stretch out your quad muscles. This helps induce the ejaculatory reflex sometimes.
As mentioned before in the “Setting up the Tens Unit” section above, you’re going to want to turn the knobs all the way to the max- 8.
Another important note:
As I mentioned earlier, I am a T8/T9 complete paraplegic. I need an extreme amount of stimulation to make it happen. If you have any feeling and/or are an incomplete injury, you can start lower on the hz. So I start at 50 hz, you could start at maybe 10 hz and go up by 10 each time. For me, ab muscle spasms are a sign it is working. I’m not an incomplete injury and I don’t have pain so it’s hard for me to tell you exactly what to do as an incomplete injury, so you will need to experiment a bit with the hz levels.
Set the “Pulse width” to the max- 300.
To begin, you’re going to start relatively LOW on the “Rate.” To access the “Rate” on your tens unit, press the “Set” button twice. You’re going to want to start your rate at 25 hz.
NOTE
Sometimes it can take only 3 sets to achieve ejaculation (around 20 minutes total). Other times it can require up to 8 sets (around 50 minutes total). Patience is required. Everyone is different. Following these protocols should work, but it will be different for different people with differing injury levels.
Set 1: 25 hz
Set the “Rate” to 25 hz on both units. Set a timer either on your Tens unit or elsewhere for 5 minutes. Once 5 minutes have passed, take a 1-2 minute break. These breaks seem to be extremely important. If you do not take a break in between tries, it likely will not happen.

Set 2: 50 hz
Set the “Rate” to 50 hz on both units. Set a timer either on your Tens unit or elsewhere for 5 minutes. Once 5 minutes have passed, take a 1-2 minute break. These breaks seem to be extremely important. If you do not take a break in between tries, it likely will not happen.

Set 3: 75 hz
Set the “Rate” to 75 hz on both units. Set a timer either on your Tens unit or elsewhere for 5 minutes. Once 5 minutes have passed, take a 1-2 minute break. These breaks seem to be extremely important. If you do not take a break in between tries, it likely will not happen.
1 to 2 Minute Break
Set 4: 100 hz
Set the “Rate” to 100 hz on both units. Set a timer either on your Tens unit or elsewhere for 5 minutes. Once 5 minutes have passed, take a 1-2 minute break. These breaks seem to be extremely important. If you do not take a break in between tries, it likely will not happen.
1 to 2 Minute Break
Set 5: 125 hz
Set the “Rate” to 125 hz on both units. Set a timer either on your Tens unit or elsewhere for 5 minutes. Once 5 minutes have passed, take a 1-2 minute break. These breaks seem to be extremely important. If you do not take a break in between tries, it likely will not happen.
1 to 2 Minute Break
Set 6: 150 hz (Max)
Set the “Rate” to 150 hz on both units. Set a timer either on your Tens unit or elsewhere for 5 minutes. Once 5 minutes have passed, take a 1-2 minute break. These breaks seem to be extremely important. If you do not take a break in between tries, it likely will not happen.
Note- During or after set 6, you can start using the Ferticare vibrator (positioning the vibrator below/underneath the tip) if you own a Ferticare. I set my ferticare to the maximum settings. So the knob at the top is set to max- 110hz, and the amplitude is set to 4.0. As I mentioned in the “risks” section. If you go to crazy with the ferticare- you push too hard or you go too long, you can cause swelling and edema. Also, if the Ferticare collides with the wires, sometimes you can get skin caught in between which can cause bleeding, so be careful. The ferticare should be positioned directly underneath the tip of the penis, and be gentle, try it on your finger and you will see why.

Set the “Rate” to 150 hz. Set a timer either on your Tens unit or elsewhere for 5 minutes. Once 5 minutes have passed, take a 1-2 minute break. These breaks seem to be extremely important. If you do not take a break in between tries, it likely will not happen.

LAST SET: Set 8: 150 hz (Max)
Set the “Rate” to 150 hz. Set a timer either on your Tens unit or elsewhere for 5 minutes. Once 5 minutes have passed, take a 1-2 minute break. These breaks seem to be extremely important. If you do not take a break in between tries, it likely will not happen.
IF IT HASN’T HAPPENED BY SET 7 OR 8, IT’S PROBABLY NOT GOING TO HAPPEN. YOUR BEST BET IS TO STOP AND TRY AGAIN ANOTHER DAY.
How to Know it is Working and How to Know it is About to Happen
Again, everyone is different. For me, when I start out a set, I can feel and see my ab muscles tense up. My legs will often spasm as well.
When I am about to ejaculate, it is very obvious that it is about to happen. The ab muscles tense up much more, the penis becomes much more erect, the legs begin to shake, and then there is about a 3 second gap in between ejaculation.
Many people ask: “Does it feel good?” My answer is: yes, it definitely feels good, and it is obviously an orgasm, but it is nowhere near as good as before.
Is it worth it? Depends. If you’re trying to get someone pregnant, obviously it is. If you’re trying to achieve an orgasm to the level you had before, don’t get your hopes up, but it DOES feel good and it is clearly an orgasm.
How to Get Someone Pregnant
You have a few options here:
- You can attempt to remove the loops during the 3 second gap you have in between realizing you’re about to ejaculate and then insert yourself inside of your significant other. I have attempted this myself and have run into issues with being able to insert in time. Also, one time I shocked my significant other on accident trying to get the loops off which she did not appreciate.
- You can attempt at home artificial insemination with a syringe kit. My wife and I call this the “turkey baster” method. There’s a link above to the one that worked for us.
- You can work with an IVF center and freeze your sperm. They will provide you a cup- you ejaculate into the cup, and bring the cup within an hour to the IVF center. IVF centers can also test your sperm quality if that’s something you’re interested in. The cost is anywhere from $500-$2000 for everything depending upon your location.
Success Rates
Laying on your back:
Before I figured this method out, when I was laying on my back, I had a success rate of around 1 out of 1000. Occasionally I could get it to work. Though it is more comfortable, I do not recommend laying on your back – at least in my case it rarely worked if ever.
In the “W” Position
Once I figured out the method I outlind above and learned about the “W” position, I was able to increase my success rate to 80-95%. Like anything else it takes practice. Everyone is different.
Time in Between Tries
For me personally, if I want to, I can do this once a day. However, I noticed if you only wait a day in between tries, sometimes it takes a lot longer and occasionally does not work. Generally I recommend waiting at least 3 days in between tries or more to let the ejaculatory reflex recharge and to give your back a rest. Also the IVF center I worked with said for semen quality and quantity reasons if you are trying to get your wife pregnant, you want to wait around 1-2 weeks in between ejaculations.
